Donruss Damaged Card Replacement Question!

So I have a Chris Carter Threads Auto thats damaged I have the Blowoutcards Recipet and my questions are,do I have to send in only one Wrapper from the box? And how long do they take to replace these things?

I think you have to fill out some form on their website first before you can submit anything. BTW, you should be getting that Inoa either tomorrow or the next day.
