Anyone have any info on this card?

i was shorted the relic from a recent box of 2010 topps pro debut which i broke. i sent the appropriate materials into topps and i received one of these in the mail today...

it is card 3 of 4 from a subset of sorts and is #/99...

what would the "appropriate materials" be? I just was shorted an auto the other week and need to do something soon

upc from the box bottom and the receipt. they also suggest sending in the empty packs but i have never bothered to do so.

I have the Beckett OPG and searched "Pedro Alvarez Topps", "Pedro Alvarez Futures Game", and simply "Pedro Alvarez" and came up empty on a memorbilia card numbered to 99. This is the closest thing I could find:
