I know it's a buyback, a 1984 Donruss serial numbered 12/15 on the back.

I have no idea what set this came out of or what year (maybe 2002?) it was inserted. Beckett.com wasn't helpful (big surprise).

2002 is correct, it appears to be this: 2002 Donruss Recollection Collection Dennis Eckersley 1984 #/15. Perhaps from 2002 Donruss Originals?

Thank you! I was hoping to use the OPG but it wasn't working for me either. I had no idea what to call this card, so thanks again I really appreciate it. I suppose the exact set isn't actually all that important.

Yeah those were randomly inserted in several Donruss. Leaf, Playoof products in the mid 2000's, I got some version Ozzie Smith from an 05 Absolute Memorabilia box!
