A few years ago the Orioles traded John Maine for Kris Benson and I remember wondering why were we trading a young guy for a guy with a history of shoulder problems.

Is it worse to go after a guy with shoulder problems or elbow problems?

Shoulder issues are WAY worse. Elbows can be fixed up nicely and actually make them stronger than before.

Although, the biggest issue with elbow injuries are the shoulder injuries they cause after recovery. The body has to re-learn the simple motor movements of the sport, so the player compensates, causing injury to the shoulder joint.

I had both at some point in my career as a pitcher. The elbow injury hurt way more, but I could still throw at the same effectiveness...it was just painful. I pitched through that injury and recovered during the offseason. The shoulder injury basically forced me to take 5-6 months off of throwing. I didn't need surgery, but I went to see a specialist and he had me solve my problem by working really hard to strengthen all my shoulder muscles. It worked. My control was gone for a little while, but it came back as I got back into throwing regularly.

Shoulder for sure

I had lots of pain in my elbow (threw curves way to early, yay me)

but then my shoulder had tendonitis, measly tendonitis, and it sidelined me for a long time (yet I didnt feel pain except when pitching)

Shoulder problems are way worse. Tommy john has somewhere around an 85% success rate, while shoulder injuries are exponentially lower. I had shoulder surgery, never regained all my velocity, only crept back up to 86 or so, and blew it out a second time. Not much fun.

Shoulder definitely. There are just more things in the shoulder that can go wrong. And when shoulder surgery is required, it is almost always are more invasive procedure than an elbow surgery. And thus, the success of shoulder surgery is lower.
