Question: Do I recall that there are some cards of players showing them in one uniform, but listing the team name as another?

IF that happens, are you going with the team name on the card or the uniform the player is in on the card as the way to determine who gets the card?

Also along those lines, what are you doing with cards of players who are in one uniform, but the game used pieces are from a former team uniform? I'd assume you are going to use the team name rather than the swatch piece, but just want to be clear so there is no confusion and no whining!
