box of 10 bowman, 09 bdp. shorted auto. where/what to send

Bought from the lcs this week, but no receipt. I know I need to mail a letter; do i need two letters for both boxes? do i need to include every wrapper?

what address do i send to? how long does it take to get something back?

+100...everytime that I've taken the time to send in for a short, I've received the worst possible replacement. '09 Unique was a Rich Hill auto. Really?!?! Not worth it.
