I would guess that it is real.

That looks like it could be the patch from the bottom of the jersey.

From my patch searches, I can remember at least 4-5 from the set that the Hamels card is in that had similar patches. If I wanted it, I would feel comfortable buying it. Plus, the weird looking thing in the upper left corner could be unique to that exact jersey that Hamels wore. Maybe he'll remember it?
