I think you're correct. He typically would make customs based on previous sets (in some cases simply scan a card, reprint it), cut out a rectangular window and be ready to rock. In this case, Doerr didn't have a 1964 Topps Stand-Up, so it's a pretty true to the original set, although instead of actual perforations, he did a nice job mimicking them.

All of that said, the auction lists it as "on card" so no window, and probably a totally legit TTM signature.

It's always easiest to compare cards from custom makers in person, but I've never seen any of his in person.
