PSA/DNA Submission form help

I have a signed card that I want to just authenticated and encapsulated. I found out the Fee for my player is $20 so have that and then $10 to cover the encapsulation. Do I have to declare a value? and what would i put for the return shipping? Any help appreciated thanks!
