Right off, let's get rid of the obvious ways.

That would be only if he could outrun the throw.

Right, but just because you make contact doesnt make it a hit.
Sorry, I thought thats what you meant, didnt realize you meant without making any contact with the ball.

It's only scored a wild pitch (100% of the time) if the runner reaches.

The first baseman picks up his glove and jogs from the dugout to first base.
The pitcher makes an out at first (such as a 3-to-1 putout).

I'm not sure, But I believe a balk can not advance the hitter - more opinions needed on that one.
