The Louisville Lip - Muhammad Ali.

The Splendid Splinter - Ted Williams.

AFAIK, he only had this as a clubhouse moniker or one that didn't 'stick':

Dirtbag - Greg Maddux. And one I prefer to 'Mad Dog'.

Godzilla - Hideki Matsui

The Flyin' Hawaiian - Shane Victorino

Shoeless - Joe Jackson

One of my all-time favorites:

The Big Unit - Randy Johnson. And IMO an excellent complement to the hilarious Topps 'prison yard' card with Pedro Martinez. First time I heard it I was like, "Geez, are they allowed to say that on tv?".

And here's one I thought was a nickname - but wasn't:

My favorite entrance/nick name was for Matt Mantei. They would play Ice, Ice Baby (Vanilla Ice) when he would make an appearance. He was always a favorite of mine.

Also, a few Chris Berman specials:
Wally "Absorbine" Joyner
Todd "Which hand does he" Frorhwirth

Denton True Young.

Two stories circulate about why they called him "Cy": One version says his pitches were so fast that the ball looked like it was coming out of a cyclone, and another version says the wooden backstop was so beat up from his pitches that it looked like it had been hit by a cyclone. Either way, he was called Cyclone for his first few years, before everyone just started calling him Cy.

In case you didn't know, Cy threw so hard that they moved back the distance between pitcher and home plate by FIVE FEET.
