Why is Porcello out after 1 inning?

In short.

Tazawa hit Cabrera (did not apper intentional).
Porcello went in on Martinez (not sure if he hit him).
Porcello then hit Youk (debatable as to whether it was intentional.)
Youk charged Porcello...both tossed.
