It holds BGS, SGC, and PSA slabs securely because the slabs actually stick up over the insert, and the foam padded top seats down upon them, holding them upright. I think cards in a toploader would move, but that could be easily fixed by laying a piece of rigid foam atop the toploaders, inside the recessed well. With the lid closed, the foam would be held in place, preventing the toploaders from moving.
