It'll cost them their 1st round pick (#14).
That draft pick is costly considering their farm system is awful. They did need someone to replace Chen so maybe they finally gave up on developing young arms and tested the free agent market instead.

Yup, the O's are notorious sticklers in their physical examinations. Could be something serious or maybe the O's are just being too picky.
It sure doesn't stop them from ruining the arms of the pitchers they draft, does it?
